Student Instructions and Agreement form for
Computer Competency Exams (form 1)


for out-of-state online students and online students who are 50 miles from the main campus only.

Please fill out this form and have your proctor fill out the (2) Proctor form  Then request an exam be sent to your proctor by filling out the (3) Request an Exam form.  Exams must be returned via e-mail immediately upon completion.  Please request all exams be sent to your proctor at the same time. 

There are three forms to fill out regardless if you have a proctor agreement with another department at WSU. Please read proctor information on Online Exams page.  Proctors cannot be a friend or family member.

The test results will be available via http://Online.Weber.Edu or Web CT VISTA approximately seven school days from the time the test was taken.  If you cannot access the grades please send an e-mail to the exam administrator requesting the score. Exam Administrator  Your full name, student ID number, and class must be included in the e-mail to receive a grade.

 

Please read and follow the instructions:

 One blank storage medium (disk/flash/CD/DVD) is needed.  If taking more than one exam, all exams may be saved on one disk.  Please create folders on the storage medium for each exam taken.  Name the folders 1501, 1502, 1503, and 1504.  Save each exam to the appropriate folder.  You will send each test via e-mail to the exam administrator immediately upon completion.  Print out and staple each completed exam to the appropriate test.  If the tests do not arrive by e-mail or cannot be opened the proctor will return the printout, disks, tests, and signed forms to the exam administrator by postal mail.  Send the completed exam(s) as an e-mail attachment to clapine@weber.edu immediately upon completing each exam. 

                      All exams are timed, closed book computer and information literacy exams. 

                      No help is to be given the student while the exam is in progress.

                      Talking or opening books will disqualify the student from the exam.

                      Sixty (60) minutes is allotted to take each exam. 

                      The Help feature of Word, Excel, PowerPoint, and Windows may be used while taking each exam with the exception of the 1504 exam. 

                      Work is to be saved often. Computers crash.  If work is not saved and the computer crashes, the student will have to start over with
no additional time.

                      If the disk/CD/Flash Drive is not readable the student must retake the exam.

                      The student is responsible for checking back with the exam administrator to see if each exam is passed.  Grades will be posted on the student online grading system located at  Web CT VISTA within one week after they are received in the exam office.  Final grades are posted to transcripts at the end of each semester.

                      Failed exams must be retaken during the semester in which the student is registered.  A $5 fee, paid to the WSU Cashier, is required for each retake.  A student may retake each exam twice.
