Forms for online students
can be accessed through the links above.
All three online forms must be
filled out regardless if you already have a proctor agreement with Continuing Education.
All three forms, Student form (1), Proctor form
(2),
and Request Exam form (3), must be completely filled out to receive
a test. The test(s) will be sent to the proctor's e-mail address.
Student's e-mail address must be included on the student form. Please read
proctor information on
Online Exams page. Proctors cannot
be a friend or family member.
Please double check your proctor's e-mail address. Grades
are recorded at http://online.weber.edu Web CT/Blackboard/VISTA. The
Grades link
explains how to access the online system.
