On-Campus Registration (use this form only after the first week of the semester when instructor approval is necessary to register)
  • This form is to be used for on-campus registration only if you want to register after the first week of school when instructor approval is required. 
    • On-campus tests are taken on the main campus in room 311 of Elizabeth Hall during the semester in which you registered.
  • This form takes the place of the printed instructor approval form for CIL exams. 
  • You can register for the tests until November 1 for fall, April 1 for spring, and July 1 for summer semester.
  • Fill out the form, if approved the form will be sent to registration, and you will be notified by e-mail when you can call to register.  Registration Phone Number: 801-626-6100
  • Print or save the form confirmation that appears when you submit the form.

  • Please look at this web site for more information about the CIL exams, as well as to locate and work the practice problems. 
  • You need your student ID or driver's license to take each exam. 
  • Exams are given in Elizabeth Hall, Room 311 each weekday up to the end of the semester. 
  • Final's week testing is from 8 to 11:00 only from Monday to Wednesday.  No testing on graduation day.   
  • You must take the tests during the semester for which you registered. 
  • If  you choose not to take a test, you will receive no credit and can register to take the test another semester.

Do not wait until the last three weeks to take your test. 
Scores will not be available.   Computers may not be available.
