Online Registration Approval Form

If you live 50 miles from campus and want to register for the online proctored exams please fill out the form below and submit to the exam administrator.  Your request will be evaluated and you will receive notice by e-mail.  If you meet the criteria you can register for the online exams by calling continuing education at 801-626-6600.  Look at the Online Registration criteria page before submitting the form.  You will receive an answer within three days.
